I wouldn't hold your breath... If it does happen it will most likely be after the game is fully completed for a second play through. A change that fundamental to the core structure of the game would likely require EVERYONE to make a full restart. Most of us have far too many hours of gameplay invested to be willing to do that. I'm sure there will be more futa/trans content introduced as we go along as additional characters are much easier to add than changing the MC and the entire game with that. We have seen the banker, our futa in law, patriarch of our dear wife Sophia attending the brothel to frequent our courtesans during our CCTV observance training during working hours and we now have a futa running our human dairy operations as well... you even get the opportunity to fuck her pussy while the doc is milking her cock for futa cum samples.
At present, Sophia is pregnant with heir number 14!!! I am current and complete on all quests to date and there is no way in hell I want to start over again. I'm pretty sure you would be missing the Doctor. I can't recall how she initially surfaces 100%, but I think you may have to try take over the 'fertility' themed business. There is only 1 such business I recall, so if you view the different business names it will stand out.
After you meet her, you can shortly after hire her from the area where you view your porfolio (if that makes sense). The doctor story ties into the farm business after building the spa. Hope I'm not confusing you.
Yes, I own a woman doctor with a complete group of 9 women, one minus, and I have completed all the game missions except this one

Had to open the game to refresh my memory but Doc is obtained when you take over the Academic District, Doc works at the Gen S Fertility Clinic until you take over the district, then she comes to work for you. You do need to build the Ranch for her which also opens up Breeding Rooms and the Milk Farm as available to purchase in the Build menu (Manage Brothel button.)
Doc was easy enough to remember because of the clinic... it's the only medical facility MC can buy out... Just a bit of supplemental info to Lulu30's post in response to Billy 25's question...
