Well the MC doesn't really have a family to bring the girls home to anymore...so there's that.

From what I understand they broke up because he decided that he didn't want to go to college, Jenn wanted to so they broke up. He ended up being pretty much forced into going to college, as he couldn't afford to live anywhere else after his parents died. His college expense would be fully provided for. Him being in college and graduating seems to be a condition of him getting his inheritance anyway.
I think the hospital comment was how he was upset that she didn't even bother to go see him or find him at the hospital. Even if they had technically broken up, she didn't even care. So for him, that's when he knew it was over and they had really "broken up"
